Each of the forms included on this page (Waiver, Photo Form and Provincial Meet Agreement) must be completed, signed by a guardian and emailed to Sharon McLay (limpy99@hotmail.com) or via fax 705-566-5011 PRIOR to the entry deadline.
Please bring payment(cash or cheque payable to District "H" Track and Field). Cost is $2 per event.
