Please open it, check all the boxes and fill in whatever information is requested (where it says sign, type your name) and submit the form. This will update your records on Trackie to indicate that you have signed the waiver. Thank you.

Refund requests:

1. Received less than 2 days before the start of a season or session will not be granted.
2. Registration refunds for medical reasons can only be processed upon receipt of a note from a medical professional. A fee will be applied for any sessions transpired.
