I understand that if my photo is taken during an event at the Mt Mac courts, it may be used on the Tennis Yukon website for promotional purposes. No names shall be used without obtaining permission from the person in the photo/his or her parent or guardian.

I authorize the Organization to collect and use personal information about me for Tennis Yukon administrative purposes only. Tennis Yukon warranties that it will not share my personal information. I understand that I may withdraw such consent at any time by contacting the Tennis Yukon Board. The Tennis Yukon Board will advise the implications of such withdrawal according to Tennis Yukon’s Privacy Policy.
