By checking this box, I agree to permit Kajaks Track & Field Club ("Kajaks") to take one or more photographs of me and/or my child(ren) and to use, reproduce, and release such images to others in print, digital, or electronic formats without payment or any other compensation to me and/or my child(ren) for any Club purposes. Such purposes may include use in Kajaks publications, posters, brochures, websites, social media, and other print or digital media for promotional, social, recreational, research, goodwill, and archival purposes. Kajaks may also provide these images to third parties that provide services to the Club, such as advertising, social media management, or event promotion, solely in connection with providing those services.

Extent of Use: I understand and acknowledge that my image, or that of my child(ren), may be viewed worldwide through the internet or in printed materials.

Usage: I agree not to commence any legal action or bring any claim against Kajaks Track & Field Club, or its employees, officials, volunteers, representatives, agents, or contractors, in connection with any matter referred to in this Photograph Waiver/Release, including, without limitation, the use, reproduction, or release of my image or that of my child(ren).

Name Usage: I also agree to the inclusion of my name and/or that of my child(ren) in connection with the uses described in this Photograph Waiver/Release.

Personal contact information will be treated as confidential, subject to statutory requirements, lawful orders, or directives, unless you provide written consent for its further release.

By providing your signature below, you acknowledge that you have read and accept this Photograph Waiver/Release and all of its terms.

Athletes aged 18 years or older may sign this waiver. For athletes under the age of 18, a parent or legal guardian must sign on their behalf.
