As the parent/legal guardian of the participant, I understand the nature of the activities involved in the C.A.N.I. Athletics Young Stars Program. I confirm that my child is physically able to participate in the program and that I am not aware of any medical or other conditions that would prevent their safe participation. I give permission for my child to take part in all training sessions, practices, competitions, and related activities organized by C.A.N.I. Athletics.
