Applicable to the EVENT: Carols by Candlelight Xmas Event at Grace Anglican Church on Friday, Dec 11/26, 19 Parkway Ave, Markham, ON

I am aware of the risks connected with attendance at any and all associated activities as noted
in Application and Specifics above. I agree to not hold accountable or bring legal action against
The Club, their officers, agents, or employees. This Waiver releases The Club from all liability
relating to injuries that may occur at a location before, during or after an activity.
By signing this agreement, I agree to hold The Club entirely free from any liability, including
financial responsibility for injuries incurred, regardless of the reasons or circumstances.
I acknowledge the risk involved with any activities I participate in, including but not limited to
muscle tears or sprains (as examples), and other physical injuries.
I certify that my participation is voluntary.
I confirm that I do not have any conditions that may increase my likelihood of injury.
I will make every effort to follow all safety rules, follow The Club personnel (Volunteer Leaders)
direction and will request any clarifications as necessary or needed.
