SAFETY / ACKNOWLEDGEMENT OF RISK
a. The responsibility for sport safety must be shared by all. I, the undersigned, am aware that there is a certain risk of injury involved in my own or my child's participation in sport, either while travelling to or from the event; or while attending or participating in the programs or activities of the events which are sanctioned/approved by BC Athletics, its Divisions, its Member Clubs or recognized organizing societies. It is understood by me that the signing of this document is intended to indicate that on behalf of myself and/or my child I assume the shared responsibility and acknowledge the risk of injury by so participating.

a. By completing this membership application form, signing and joining BC Athletics you consent to the collection of this information and its use as per the BC Athletics Privacy Statement and Policy - see Identifying Purposes - Appendix II of the Policy available at www.bcathletics.org

CANADIAN ANTI-DOPING PROGRAM (CADP)
The Canadian Anti-Doping Program (CADP) is generally considered to be one of the most thorough and comprehensive doping control policies in the world. By operating on the basis of a collective agreement within the sport, the policy provides for a truly independent, transparent and cost-effective anti-doping system. It also provides the procedural fairness and protects the athletes’ rights throughout the process.
Benefits to Athletics Canada and its Members:
Independent, transparent system provides the athletes and the public, confidence in Athletics Canada and our anti-doping policy.
When all (majority) sports adopt the policy, it reduces our liability. The CADP serves as a shield against legal challenges to individual National Sport Organizations (NSO).
The CADP and more specifically, the Standard Operating Procedures have not had any successful legal challenges.
All testing services, laboratory analysis, result management and the administration of procedural fairness are all paid for by the Federal Government and the Canadian Centre for Ethics in Sport when Athletics Canada becomes part of the system by adopting the CADP.
Athletics Canada has adopted the 2015 Canadian Anti-Doping Program as the anti-doping policy and regulations of the organization.
Sea to Sky Athletics Liability Release
In consideration of your accepting my/my child's application for membership in the Sea to Sky Athletics, I forever release and discharge the Sea to Sky Athletics, it's directors, officers, coaches, employees, agents and volunteers ("Sea to SKy Athletics Group") from any and all actions, damages, claims, demands, costs and
expenses whatsoever which might arise by reason of traveling to or from or participating in training sessions or competitions whether or not incurred by the negligence of any Sea to Sky Athletics, and I agree to indemnify and save harmless Sea to Sky Athletics, their personal representatives, successors and assigns against and from all actions, damages, claims, demands, costs and expenses which may hereafter be brought or made against them or any of them by or on behalf of myself/my child because of traveling to or from or participating in training sessions or competitions whether or not incurred by the negligence of any of the Seat to Sky Athletics, except to the extent and amount covered by accident or liability insurance or both.
The Sea to Sky Athletics asks your permission to take one or more photographs of you and/or your child to use. By signing this release/waiver you are agreeing to the terms set out in this form.
To the best of my knowledge the above listed athlete is in good health and the medical information is accurate.
Photo Release

I agree to permit Sea to Sky Athletics to take one or more photographs of me and/or my child(ren) to use, reproduce and release to others such images whether in print, digital or electronic formats without payment or any other compensation to me and/or my child(ren), for any Sea to Sky Athletics purposes. Such purposes may include using them in Sea to Sky Athletics publications, posters, brochures, online/Internet or other media for promotional, social, recreational purposes. Sea to Sky Athletics may also provide these images to a third party providing a service to Sea to Sky Athletics for use on advertising, social media net working or event promotion. I agree and acknowledge that my image or that of my child may be seen on the worldwide via the Internet or printed media.
