I, acknowledge that participation in Sport Canada activities, including but not limited to Sport Canada events, involves certain risks and dangers that cannot be eliminated, even with careful planning and instruction. I understand that these risks and dangers may result in bodily injury, death, or property damage.

In consideration of being allowed to participate in Sport Canada activities, I, on behalf of myself and my heirs, executors, administrators, and assigns, agree to assume all risks and accept full responsibility for any and all injuries, damages, or losses that I may sustain as a result of participating in these activities.

I hereby release, waive, discharge, and covenant not to sue Sport Canada, its directors, officers, employees, volunteers, agents, representatives, and assigns (collectively referred to as “Releasees”), from any and all liability, claims, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any property belonging to me, whether caused by the negligence of the Releasees or otherwise, while participating in [name of sports club] activities.

I understand and acknowledge that Sport Canada does not provide insurance coverage for my participation in these activities, and that I am responsible for obtaining my own insurance coverage.

I certify that I am physically fit to participate in Sport Canada activities, and that I have not been advised otherwise by a qualified medical professional.

I understand that this Waiver of Liability is intended to be as broad and inclusive as permitted by law, and that if any provision of this Waiver of Liability is held to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable.

I have read this Waiver of Liability, fully understand its terms, and voluntarily sign it, with the understanding that I am giving up substantial rights by doing so.
