RUN FOR LIFE – PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND PHOTO CONSENT (UNDER 16)

ASSUMPTION OF RISK

I, the undersigned parent/guardian, understand that participation in the Run for Life Dandelion Girls Camp involves physical activity, including running, games, and outdoor activities. I acknowledge that these activities carry inherent risks, including but not limited to falls, collisions, weather-related conditions, and other unforeseen risks.

I confirm that my child is physically able to participate in camp activities and agree that participation is voluntary.

RELEASE OF LIABILITY

In consideration of my child being permitted to participate, I hereby release and hold harmless Run for Life, its directors, staff, volunteers, coaches, and partners from any and all claims, demands, damages, or liability arising out of or in connection with my child’s participation in the camp.

This includes any injury, loss, or damage sustained during camp activities, except where caused by gross negligence.

MEDICAL CONSENT

I authorize Run for Life staff and volunteers to obtain emergency medical care for my child if necessary. I understand that every effort will be made to contact me in the event of an emergency.

CODE OF CONDUCT

I understand that my child is expected to participate respectfully and safely. Run for Life reserves the right to remove a participant if their behavior is unsafe or negatively impacts others.

PHOTO & MEDIA RELEASE

I grant permission for Run for Life to take photographs and/or videos of my child during camp activities and to use these images for promotional purposes, including social media, website, and printed materials.

ACKNOWLEDGEMENT & DIGITAL CONSENT

By checking the box during registration, I confirm that I am the parent/guardian of the participant and that I have read, understood, and agree to the terms of this waiver.

I understand that by agreeing electronically, I am providing the equivalent of a legal signature and am giving up certain legal rights on behalf of my child.
